Medical Report (MF1 form) | You will need to provide their birth certificates if you are accompanied by children, adopted or stepchildren, or your parents. Applicants with children approaching the age of 21 must submit their applications at least 6 months before to their children's 21st birthday. If you are travelling with impaired children above the age of 21, you should obtain a Letter of Confirmation from a medical specialist or general practitioner. As the principal applicant, you must provide a Statutory Declaration to bear all expenses and financial requirements during the stay in Malaysia for dependants. | Dependents | |
